
Do not decide from colour alone
An unexpected result needs assessment against the exact product instructions and the facility’s release procedure. Keep the affected items from use pending that assessment. Do not treat a second successful indicator as an explanation for the first failure.
Capture a clear incident record
Record the SKU, lot, expiry, storage history, load identifier, placement, programme and cycle record. Photograph the original result under neutral lighting alongside the manufacturer’s reference, where available. Describe what was observed rather than labelling it a manufacturing defect before investigation.
Send an answerable technical enquiry
Ask the supplier to confirm product compatibility and the interpretation of the observed endpoint. Share the documented facts with the responsible sterilization team. Record the investigation outcome and authorized disposition so that the incident remains linked to the original load.
